~CARE COORDINATOR: Pathways to Success–Mental Health Services

Join our team as we GROW!!! We’re starting a new program and are looking for Care Coordinators to provide high quality care coordination, crisis stabilization, community support, and discharge planning services to children who have mental illness. You must be able to form positive working relationships with other service providers and family members in order to best support the children in this program. Services will be provided in the clients’ natural environment (home, community) within Adams and surrounding counties. Applicants must have:

  • BA/BS degree in Psychology, Social Work, or other Human Services field
  • Valid driver’s license, reliable transportation, and a good driving record

Full-time position! Primarily daytime hours of work Monday – Friday with two evenings/week and flexibility to accommodate unplanned client/family needs with little notice.

~CLINICAL MANAGER–Mental Health Services        **NEW POSITION!**

We’re looking for a Licensed Clinical Social Worker (LCSW) or a Licensed Clinical Professional Counselor (LCPC) who’s ready for an opportunity to grow professionally and help us kick off a new, exciting program serving children who have complex behavioral health needs. In this position you’ll get to provide leadership and supervision for a new program from the ground, up! Parent Educators empower parents to be their child’s first teacher using the Parents as Teachers evidenced-based practice model. They provide free educational and developmental tools to support parents, complete assessments, treatment plans, and deliver case management services.  Full-time position, mainly daytime and early evening hours of work according to parents’ needs along with the employee’s preferences, providing a flexible work schedule.

This position requires a Bachelor’s Degree in Early Childhood Education or a Human Services field and experience with young children is preferred.

~EMPLOYMENT SPECIALIST–Mental Health and DD Services

Our Employment Specialists provide high quality placement and supportive services to adults who have mental illness or a disability. They form productive relationships with their clients and community employers that result in clients meeting their vocational goals for paid work in the community. This work is performed using an evidenced-based practice in the client’s natural environment (home and community). Primarily daytime hours of work, plus participation in an after-hours crisis on-call system.

Requires BA/BS in Psychology, Social Work, or Human Services. Experience in mental health/recovery services or job placement is preferred.



We’re looking for a Human Resources Specialist to perform HR functions working with a staff of about 200 employees. Responsibilities include managing all aspects of staff training (scheduling, enrolling, tracking completion, reporting, etc.), scheduling and conducting interviews for vacant positions, checking references, coordinating the agency’s health and safety functions, and maintaining systems to track multiple projects. Will also perform other duties within the scope of Human Resources or to support the agency’s employees and mission.

A Bachelor’s Degree in Human Resources, Business Management, Communications, or similar field of study is required; work experience in HR is preferred. Excellent communication skills, friendly personality, the ability to change gears/priorities quickly and to navigate multiple projects simultaneously are very important in this role.  Full-time position, daytime hours of work, Monday through Friday.

Our Mobile Crisis Specialists respond onsite to people in crisis in a 3-county area as well as provide short-term follow-up stabilization and care coordination services.  This unique position is on-call evenings –> overnights 2-3 times/week from the comfort of home with a 4-day workweek and 3-day weekends every third week!  This is a full-time, exempt position with a minimum of 35 hours/week, while paid for 40 hours/week.  Perfect position for a ‘night owl’ who enjoys helping people problem-solve and frequent days off!

As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others this might be the job for you!  CNA experience is helpful but paid training is provided too. Our evening/weekend and overnight positions feature competitive pay and two bonuses paid during the first year!   Must be 18+ years of age and possess a High School Diploma/GED. We’re currently looking to hire for these shifts/hours:

  • Full-time or Part-time; Evenings/Weekends–CILA Group Home
  • Part-time; Weekends Only–CILA Group Home
  • Full-time; Evenings and every other Weekend–Community Villa (must be at least 21 years of age for this position)
  • Full-time; Weekdays–New Horizons program
  • Full-time; Combination of Day, Evening, and every other Weekend shifts–Intermittent CILA Services

Full-time evening/weekend and overnight positions feature a 4-day work week!
