COMMUNITY SUPPORT WORKER–Mental Health Services

NEW POSITION!!!  who will provide community support, counseling, skills training, and stabilization services to guests at our ‘Living Room’ and individuals living in the community who are in need of mental health services. CSWs will assess guests’ need for services and provide recovery-oriented education and linkage to community resources and natural supports. This work will be conducted at the Living Room and in natural environments (home and community).  Applicants must have a Bachelor’s Degree in Psychology, Social Work, or other Human Services field.  Hours are daytime with 2 weekday evenings/week until 8pm and on-call coverage about once/week.  Full-time position.

HOUSING SPECIALIST–Mental Health Services

DIRECT SUPPORT PROFESSIONAL–Developmental/Intellectual Disability Services

As a Direct Support Professional at Transitions, you get to work with intellectually or developmentally disabled consumers in home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others this might be the job for you!  CNA experience is helpful and  paid training is provided. Must be 18+ years of age and possess a High School Diploma/GED. We’re currently looking to hire for these shifts/hours:

  • Full-time Overnights–4-day workweek (or could be part-time, 3 days/week  if candidate preferred)
  • Full-time Evenings and Every Other Weekend–4-day workweek
  • Part-time Days–Monday through Friday, no weekends
