~RN SUPERVISOR OF NURSING & PSYCHIATRY SERVICES —Mental Health Services

In this role, you will oversee psychiatric nursing and provider services at a fast-paced, outpatient behavioral health agency. This role combines clinical leadership, staff supervision, program management, quality oversight, and collaboration with psychiatric providers to ensure exceptional care and positive outcomes for the individuals we serve.
Duties
- Lead and supervise outpatient nursing and contracted psychiatric services within a community behavioral health organization.
- Provide clinical supervision and support to all agency LPNs and serve as a key liaison to the CCBHC Medical Director.
- Oversee staff development, program operations, quality outcomes, regulatory compliance, and service excellence.
- Drive program growth, quality improvement initiatives, and integrated behavioral health care services.
- Ensure fiscally responsible operations while supporting a high-performing, consumer-centered workforce.
Requirements:
- RN licensure in Illinois
- 5 years of experience in behavioral health/psych
- 3 years of experience in supervision/management
This is a full-time, daytime position (Monday – Friday) with an on-call component that is shared with another RN with call response mainly by phone.

~COMMUNITY SUPPORT WORKER —Mental Health Services and Developmental/Intellectual Disability Services

As a Community Support Worker, you’ll work one-on-one with adults living with mental illness in their homes and communities, helping them build the skills they need to live more independently. This role calls for someone who is compassionate, self-motivated, and organized — someone who can build genuine relationships with clients while also staying on top of the documentation that supports their care.
This position requires:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver’s license, reliable vehicle, and a good driving record
- Ability to pass fingerprint background checks and drug test
- Work in natural settings such as in homes or in the community
- After-hours on-call coverage 3-4 times/month
- Must reside within 30 minutes of Quincy, IL
Full-time position with competitive base pay, bonus program, and mileage reimbursement. Pay range $50,500.00 – $53,000.00 per year (dependent upon experience).

Our Therapists provide mental health services in an outpatient setting using collaborative treatment approaches tailored to suit their own style as well as the needs of the clients they’re serving. Therapists at Transitions serve all populations including children, adults, couples, and families mainly in our busy, outpatient mental health setting. Partnering with other community agencies, medical providers, or schools to provide onsite therapy services may be necessary in this position.

~CARE COORDINATOR, Pathways to Success —Mental Health Services
*This is a Work-From-Home position after initial training period. Applicants must live within 30 miles of Quincy, IL to be considered for this position.*
We’re growing! Join our team as a Pathways to Success Care Coordinator and help expand services to children/families who have complex behavioral health needs and require intensive services and support. Using a child and family team wrapround planning process, Pathways Care Coordination makes a real difference for youth and families by developing a family vision, identifying their needs, and connecting them to needed services.
With a manageable caseload of 10-16 children, you’ll have the time to build strong, positive, working relationships with children, families, and other service providers.
This position requires:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver’s license, reliable vehicle, and a good driving record
- Ability to pass fingerprint background checks and drug test
- Work in natural settings such as homes, schools, and/or in the community in Adams County and surrounding counties
One year experience as a Mental Health Professional is preferred and experience working with children/adolescents is highly preferred.
Full-time position! Hours of work are primarily daytime, Monday – Friday, with two evenings/week and schedule flexibility to meet with children/families at times that work for them. This position isn’t required to participate in our after-hours on-call rotation either! This results in Care Coordinators being able to have flexible work schedules based on their personal or family needs too.
Pay range $53,248 – $54,080/year (dependent upon experience) with mileage reimbursement as well.

Are you passionate about having a direct impact on the lives of people who are experiencing a crisis? Consider this unique position! You will be on-call evenings/overnights two times per week on a rotating schedule, largely from the comfort of home. Your other two shifts are daytime on-call shifts to comprise a 4-day workweek. Weekend on-call is part of your rotating schedule as well. After receiving a call about a child or adult who is in a mental health crisis, you’ll respond to the site of the crisis in a 5-county area to conduct a screening. Our Mobile Crisis Specialists also provide short-term follow-up stabilization and care coordination services.
This is a full-time, exempt position with a minimum of 35 hours/week while paid for 40 hours/week. It’s the perfect position for a ‘night owl’ who enjoys helping people problem-solve and enjoys having frequent days off!

~DIRECT CARE STAFF (DIRECT SUPPORT PROFESSIONAL)–Developmental/Intellectual Disability or Mental Health Services

As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others, this rewarding work might be for you! CNA experience is helpful; however, paid training is provided. Our evening/weekend and overnight positions feature competitive pay along with two bonuses paid during the first year! Requirements: must be 18+ years of age, possess a High School Diploma/GED, and possess valid driver’s license and good driving record.
