This is your opportunity to lead a dedicated, program-specific team—not juggle multiple departments—while helping ensure children with mental health challenges receive the coordinated care they deserve!
As Program Supervisor, you’ll support and guide a team of committed care coordinators who provide community-based services in a 6 county area. You’ll have the chance to grow professionally, develop leadership skills, and make a meaningful impact in the lives of youth and families (no therapy duties required). All of this while working in a supportive, mission-driven environment where collaboration and compassion come first!
Applicants must have:
- Master’s Degree in Social Work, Counseling, or other Human Services field; LCSW or LCPC preferred.
- 2+ years of supervisory/management experience preferred
Full-time position, primarily daytime hours of work with occasional after-hours on-call rotation from the comfort of your home (with extra pay!) .
Pay range $33.47 – $40.79/hour dependent upon experience and/or licensure. Now offering $3,000 sign-on bonus for unlicensed candidate; $5,000 sign-on bonus for LCSW/LCPC!

~CARE COORDINATOR, PATHWAYS TO SUCCESS —Mental Health Services

We’re growing! Join our team as a Pathways to Success Care Coordinator and help expand services to children/families who have complex behavioral health needs and require intensive services and support. Using a child and family team wrapround planning process, Pathways Care Coordination makes a real difference for youth and families by developing a family vision, identifying their needs, and connecting them to needed services.
With a manageable caseload of 10-16 children, you’ll have the time to build strong, positive, working relationships with children, families, and other service providers.
This position requires:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver’s license, reliable vehicle, and a good driving record
- Ability to pass fingerprint background checks and drug test
- Work in natural settings such as homes, schools, and/or in the community in Adams County and surrounding counties
One year experience as a Mental Health Professional is preferred and experience working with children/adolescents is highly preferred.
Full-time position! Hours of work are primarily daytime, Monday – Friday, with two evenings/week and schedule flexibility to meet with children/families at times that work for them. This results in Care Coordinators being able to have flexible work schedules based on their personal or family needs too!
This position isn’t required to participate in our after-hours on-call rotation; rather, you would respond if a child on your caseload has a crisis during your work hours or, if after-hours, may be called upon for consultation about the child.
Pay range from $25.60 – $26.00/hour with generous mileage reimbursement rate.

~DIRECT CARE STAFF (DIRECT SUPPORT PROFESSIONAL)–Developmental/Intellectual Disability or Mental Health Services
** NEW** $400 Sign-on Bonus for all DSPs!
As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others, this rewarding work might be for you! CNA experience is helpful; however, paid training is provided. Our evening/weekend and overnight positions feature competitive pay along with two bonuses paid during the first year! Requirements: must be 18+ years of age, possess a High School Diploma/GED, and possess valid driver’s license and good driving record.
We’re currently looking to hire for these shifts/hours:
- Part-time: Evening/Weekend shifts–CILA Group Home [Pay: From $19.50/hour]
