~EMPLOYMENT SPECIALIST —Mental Health Services and Developmental/Intellectual Disability Services
Our Employment Specialists get to connect adults who have a disability to jobs in the community by forming productive relationships with their clients and with local employers! This work is performed using a highly successful, evidenced-based practice in the client’s natural environment (home and community). This is a full-time position with primarily daytime hours of work plus occasional evening or weekend hours in order to support clients who have jobs in the community.
- Requires BA/BS degree in Psychology, Social Work, or Human Services. Experience in Mental Health, DD/ID services, or job placement is preferred. Pay range $24.31 – $25.50/hour (dependent upon experience).
~CARE COORDINATOR, Pathways to Success —Mental Health Services
*This is a Work-From-Home position after initial training period. Applicants must live within 30 miles of Quincy, IL to be considered for this position.*
We’re growing! Join our team as a Pathways to Success Care Coordinator and help expand services to children/families who have complex behavioral health needs and require intensive services and support. Using a child and family team wrapround planning process, Pathways Care Coordination makes a real difference for youth and families by developing a family vision, identifying their needs, and connecting them to needed services.
With a manageable caseload of 10-16 children, you’ll have the time to build strong, positive, working relationships with children, families, and other service providers.
This position requires:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver’s license, reliable vehicle, and a good driving record
- Ability to pass fingerprint background checks and drug test
- Work in natural settings such as homes, schools, and/or in the community in Adams County and surrounding counties
One year experience as a Mental Health Professional is preferred and experience working with children/adolescents is highly preferred.
Full-time position! Hours of work are primarily daytime, Monday – Friday, with two evenings/week and schedule flexibility to meet with children/families at times that work for them. This position isn’t required to participate in our after-hours on-call rotation either! This results in Care Coordinators being able to have flexible work schedules based on their personal or family needs too.
Pay range from $25.60 – $26.00/hour with mileage reimbursement.
Requirements:
- Current LPN license
- Associate’s Degree or 60 college credit hours to qualify as Paraprofessional in IL
- Ability to pass criminal background checks and a drug test
Full-time position with daytime hours of work Monday through Friday (about 35 hrs/wk). This position could also be a part-time position for a candidate desiring fewer, daytime hours of work with non-bedside, low-acuity nursing duties.

~DIRECT CARE STAFF (DIRECT SUPPORT PROFESSIONAL)–Developmental/Intellectual Disability or Mental Health Services

As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others, this rewarding work might be for you! CNA experience is helpful; however, paid training is provided. Our evening/weekend and overnight positions feature competitive pay along with two bonuses paid during the first year! Requirements: must be 18+ years of age, possess a High School Diploma/GED, and possess valid driver’s license and good driving record.
We’re currently looking to hire for these shifts/hours:
- Part-time: Evening/Weekend shift–CILA Group Home [Pay: from $20.00/hour]
- Part-time: Daytime shift with occasional after-hours on-call – Day Program [Pay: from $18.50/hour]
- Full-time Float: Must be able to work evenings/weekends regularly and overnights on an as-needed basis – CILA Group Homes [Pay: from $21.50/hour]
