PEER RECOVERY SPECIALIST–Substance Abuse Services ***NEW POSITION!***
This valuable new position will provide recovery-oriented, peer-based individual and group services to consumers who have substance use disorder, mental illness, or are dually diagnosed. Our Peer Recovery Specialist will contribute essential expertise to consumers in their recovery process and utilization of natural supports, assist with connection to community resources, and perform outreach and collaboration with other providers/community partners.
Applicants must be 21+ years of age, possess a Bachelor’s Degree in a Human Services field (Psychology, Social Work, etc.), have experience as a personal recipient of substance abuse services, and demonstrate the ability to incorporate his/her own direct experience in their recovery journey in order to inspire others.
DIRECT SUPPORT PROFESSIONAL–Developmental/Intellectual Disability Services
As a Direct Support Professional at Transitions, you get to work with intellectually or developmentally disabled consumers in home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others this might be the job for you! CNA experience is helpful and paid training is provided. Must be 18+ years of age and possess a High School Diploma/GED. We’re currently looking to hire for these shifts/hours:

- Full-time Evenings/Weekends–CILA Group Home

THERAPIST–Mental Health Services
Join our team of mental health professionals who help individuals, couples, and families improve their lives every day! Our Therapists provide mental health services in outpatient and community-based settings using a collaborative treatment approach. Full-time position, primarily daytime hours of work plus on-call coverage. Requires a Master’s Degree in Social Work, Counseling, or other Human Services field; LCSW or LCPC preferred. SPECIAL EDUCATION TEACHER–Developmental/Intellectual Disability Services
Our Special Education Teachers make a difference in the lives of children who have multiple disabilities and autism by implementing interactive IEPs which include educational and therapeutic goals and activities. Our private school has high staff-to-student ratios and a great environment fueled by a spirit of teamwork in the best interests of the children. This is an 11-month position (regular school year plus 5-week summer school); paid bi-weekly, year-round. Requires Bachelor’s Degree in Special Education and Illinois LBS-1 teaching certificate (or able to obtain).
MOBILE CRISIS SPECIALIST–Mental Health Services
Our Mobile Crisis Specialists respond onsite to people in crisis in a 3-county area as well as provide short-term follow-up stabilization and care coordination services. This unique position is on-call evenings/overnights 3 times/week from the comfort of home and will have some afternoon or early evening hours of work Monday –> Friday, along with 3 days off every other weekend. This is a full-time, exempt position with minimum 35 hours/week, but paid at 40 hours/week. Applicants must have a BA/BS degree in Psychology, Social, Work, or other Human Services field.
CHILD & ADOLESCENT CASE MANAGER–Mental Health Services
Child & Adolescent Case Managers make a real difference to children and their families every day by providing high quality crisis stabilization and case management services. They work with children who have experienced a psychiatric crisis by providing assessment and helpful supports, coordinating services with community providers, and teaching valuable skills. Applicants must have a Bachelor’s Degree in Psychology, Social Work, or other Human Services field. Hours are daytime/early evenings and after-hours on-call coverage 2-3 times/month. Full-time position.
COMMUNITY SUPPORT WORKER–Mental Health Services
A Community Support Worker at Transitions provides valuable case management, skills building, counseling, community support, and stabilization services to adults who have serious mental illness. These services are provided in the clients’ natural environment (home, community) within Adams County. Applicants must have a Bachelor’s Degree in Psychology, Social Work, or other Human Services field. Hours are primarily daytime with after-hours on-call coverage 2-3 times/month. Full-time position.
JANITORIAL EMPLOYEES–Developmental/Intellectual Disability Services
Our Janitorial Employees perform janitorial and housekeeping duties such as mopping, cleaning restrooms, emptying trash, dusting, vacuuming, and cleaning windows in business or industrial settings as part of a client job training program. Hours of work are part-time, Monday through Friday on either a morning-to-early afternoon crew or an evening crew; no weekends.
All positions require ability to pass background checks and a drug test. Most positions require reliable transportation, a valid driver’s license, and a good driving record.
