~RECOVERY SUPPORT SPECIALIST —Mental Health Services

A Recovery Support Specialist at Transitions works as part of our Community Support Services team to provide recovery-oriented, community-based services to adults who have serious mental illness.  Our Recovery Support Specialists use their own personal recovery experience in mental health services to educate others and to instill hope.

Candidates for this position must be 21+ years old, possess a HS Diploma/GED, and must have direct, personal experience as a recipient of mental health services/treatment.

Full-time position, Monday – Friday, primarily daytime hours, with availability to work one evening/week to meet client needs.

Pay range $18.03 – $18.93/hour (dependent upon experience).

 

~ACCESS SPECIALIST– Mental Health Services

As an integral part of our team in a busy outpatient mental health clinic, our Access Specialists play an important role in assisting people with access and linkage to our services by completing crisis intervention, initial assessment, registration services, and care coordination responsibilities. Duties include:

  • Registration and orientation to services for clients
  • Crisis intervention/stabilization services
  • Initial assessment and treatment planning
  • Screening and linkage to services; referral to community resources
  • Coordinating client access to psychiatry services
  • Scheduling appointments
  • Care coordination
  • Data entry, phone consultation, insurance verification, and other office duties

Requires Bachelor’s Degree in Psychology, Social Work, Human Services, or other Human Services field from an accredited college or university.  Strong engagement and interpersonal skills and flexibility are also required in this position.  Experience in the mental health field is preferred.

Full-time position, Monday through Friday, daytime hours of work.  After-hours on-call is not required in this position.

Pay range $24.91 – $26.15/hour ($51,800 – $54,392/year), dependent upon experience.

 

~SCHOOL PARAPROFESSIONAL--Developmental/Intellectual Disability Services 

Do you want to make a difference in the lives of children? Great! We’re opening a new classroom in August of 2026 and are looking for Paraprofessionals to work with children who have autism and related communication disorders at our private school. Our Paras get to work with children individually or in small groups to implement their IEPs, perform ADLs, and participate in community-based outings with the students.

Transitions School features high staff-to-student ratios, amazing students, a collaborative team work environment, and competitive pay!

Requirements:

  • Ability to work as part of a great team
  • Must be ISBE Certified Paraprofessional or have 60 hours of college credits in any course of study
  • Ability to pass criminal background checks and a drug test

Daytime hours of work Monday through Friday (about 35 hrs/wk), generous paid holidays, health, dental, and life insurance, AFLAC plans, sick time accrual, and a retirement plan with up to 4% employer match are just a few of the benefits available to you in this position. Pay range $17.60 – $19.36/hour, dependent upon experience.

~GROUP HOME MANAGER —Mental Health Services

In this role, you’ll oversee daily operations of our 15-bed residential home, lead a team of direct care staff, and serve as a trusted presence for residents and families alike — all while working with a high degree of independence.

Full-time position, primarily daytime hours of work with some after-hours on-call responsibility by phone.  Applicants must have:

  • BA/BS in Social Work, Psychology, or other Human Services field
  • 2+ years of supervisory experience
  • Valid driver’s license and a good driving record
  • Ability to pass fingerprint background checks and drug test

Pay range $59,000 – $61,000/year (dependent upon experience) plus incentive program, mileage reimbursement, and great employee benefits are available!

~YOUTH COMMUNITY SUPPORT WORKER —Mental Health Services

Our Youth CSWs make a real difference to children who have experienced a psychiatric or behavioral crisis by providing assessment and helpful supports, coordinating services with community providers, and teaching valuable skills.  This position requires:

  • Bachelor’s Degree in Psychology, Social Work, or other Human Services field
  • After hours on-call coverage 3-4 times/month

This is a full-time position with flexible scheduling to meet our clients’ as well as your own needs. Competitive base pay, incentive program, mileage reimbursement, and great benefits await you in this position!  Pay range $24.91 – $26.15/hour ($51,800 – $54,392/year), dependent upon experience.

~SCHOOL NURSE/LPN--Developmental/Intellectual Disability Services
Wanting to make a difference in the lives of children? We’re looking for a School Nurse to work with children who have autism and related communication disorders at our private school. Our School Nurse provides nursing care for school students along with general clerical duties to support the school staff in order to meet the needs of students. Also serves as a Float Paraprofessional in classrooms working with students to implement their IEPs, perform ADLs, and participate in community-based activities. Our small, private school serves 15-20 students in three classrooms and features high staff-to-student ratios.

Requirements:

  • Current LPN license
  • Associate’s Degree or 60 college credit hours to qualify as Paraprofessional in IL
  • Ability to pass criminal background checks and a drug test

Full-time position with daytime hours of work Monday through Friday (about 35 hrs/wk). This position could also be a part-time position for a candidate desiring fewer, daytime hours of work with non-bedside, low-acuity nursing duties.

~DIRECT CARE STAFF (DIRECT SUPPORT PROFESSIONAL)–Developmental/Intellectual Disability or Mental Health Services

As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others, this rewarding work might be for you!  CNA experience is helpful; however, paid training is provided. Our evening/weekend and overnight positions feature competitive pay along with two bonuses paid during the first year!  Requirements:  must be 18+ years of age, possess a High School Diploma/GED, and possess valid driver’s license and good driving record.

We’re currently looking to hire for these shifts/hours:

  • Part-time: Evening/Weekend shift–CILA Group Home    [Pay: from $20.00/hour]
  • Full-time and Part-time: Daytime shift with occasional after-hours on-call – Day Program  [Pay: from $18.50/hour]
  • Full-time: Overnight shift–CILA Group Home    [Pay: from $21.50/hour]
