RESIDENTIAL PROGRAM MANAGER; QIDP
A Residential Program Manager at Transitions gets to make a real difference in the lives of adults who have an intellectual or developmental disability by overseeing services they receive in a group home setting. Every day you will have the opportunity to enrich the lives of the residents who live in the home by providing engaging activities, a safe, clean home environment, and well-trained employees to support them. This is a full-time position, mainly daytime hours of work, with occasional on-call rotation.
2+ years of experience in supervision and a degree in a Human Services field (Psychology, Social Work, Special Education, etc.) is required. QIDP status preferred.
Our Parent Educators provide valuable home visiting services for parents of children ages Birth to Three. According to our participants, this is one of our most well-loved programs! Parent Educators equip parents with free educational and developmental tools using the Parents as Teachers evidenced-based practice model, along with completing assessments, treatment plans, and case management services. Part-time or full-time position, mainly daytime and early evening hours of work according to parents’ needs along with the employee’s preferences, providing a flexible work schedule.
This position requires a Bachelor’s Degree in Early Childhood Education or a Human Services field and experience with young children is preferred.
DIRECT SUPPORT PROFESSIONAL–Developmental/Intellectual Disability Services
As a Direct Support Professional at Transitions, you get to work with intellectually or developmentally disabled consumers in home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others this might be the job for you! CNA experience is helpful and paid training is provided. Must be 18+ years of age and possess a High School Diploma/GED. We’re currently looking to hire for these shifts/hours:

- Full-time Evenings/Weekends–CILA Group Home
- Part-time Evenings/Weekends–CILA Group Home
- Weekends Only–CILA Group Home
THERAPIST–Mental Health Services
Join our team of mental health professionals who help individuals, couples, and families improve their lives every day! Our Therapists provide mental health services in outpatient and community-based settings using a collaborative treatment approach. Full-time position, primarily daytime hours of work plus on-call coverage.
This position requires a Master’s Degree in Social Work, Counseling, or other Human Services field; LCSW or LCPC preferred. Competitive pay, bonus opportunity, and excellent benefits await you! We are also an approved site for the National Health Service Corps(NHSC) which grants loan forgiveness for LCSWs and LCPCs.
SPECIAL EDUCATION TEACHER–Developmental/Intellectual Disability Services
Our Special Education Teachers make a difference in the lives of children who have multiple disabilities and autism by implementing interactive IEPs which include educational and therapeutic goals and activities. Our private school has high staff-to-student ratios and a great environment fueled by a spirit of teamwork in the best interests of the children.
This is an 11-month position (regular school year plus 5-week summer school); paid bi-weekly, year-round. Requires Bachelor’s Degree in Special Education and Illinois LBS-1 teaching certificate (or able to obtain).
MOBILE CRISIS SPECIALIST–Mental Health Services
Our Mobile Crisis Specialists respond onsite to people in crisis in a 3-county area as well as provide short-term follow-up stabilization and care coordination services. This unique position is on-call evenings/overnights 3 times/week from the comfort of home and will have some afternoon or early evening hours of work Monday –> Friday, along with 3 days off every other weekend. This is a full-time, exempt position with minimum 35 hours/week, but paid at 40 hours/week.
Applicants must have a BA/BS degree in Psychology, Social, Work, or other Human Services field.
COMMUNITY SUPPORT WORKER–Mental Health Services
A Community Support Worker at Transitions provides valuable case management, skills building, counseling, community support, and stabilization services to adults who have serious mental illness. These services are provided in the clients’ natural environment (home, community) within Adams County. Hours are primarily daytime with after-hours on-call coverage 2-3 times/month. Full-time position.
Applicants must have a Bachelor’s Degree in Psychology, Social Work, or other Human Services field.
All positions require ability to pass background checks and a drug test. Most positions require reliable transportation, a valid driver’s license, and a good driving record.
