~QIDP (CASE MANAGER)– Developmental/Intellectual Disability Services
Looking for a rewarding professional position? We recruiting a caring, organized QIDP (Case Manager) to coordinate services for, advocate for, and work directly with adults who have intellectual/developmental disabilities in this new position! Full-time position, Monday – Friday with occasional on-call coverage. Daytime – early evening hours of work.
Candidate must have a BA/BS degree in Psychology, Social Work, Human Services, Recreation, Special Education, or Rehab Counseling. Pay range $20.69 – 21.72/hr (dependent upon experience).
Our Employment Specialists get to connect adults who have mental illness or a disability to jobs in the community by forming productive relationships with their clients and local employers! This work is performed using a highly successful evidenced-based practice in the client’s natural environment (home and community). This is a full-time position with primarily daytime hours of work plus participation in an after-hours crisis on-call system a few days/month.
Requires BA/BS degree in Psychology, Social Work, or Human Services. Experience in mental health/recovery services or job placement is preferred. Pay range $23.81 – $25.00/hour (dependent upon experience).

~CARE COORDINATOR —Mental Health Services
Join our team as we GROW!!! We’re looking for Care Coordinators to provide high quality care coordination, crisis stabilization, community support, and discharge planning services to children who have mental illness. You must be able to form positive working relationships with other service providers and family members in order to best support the children served in this program. Services will be provided in the clients’ natural environment (home, community) within Adams County and surrounding counties. Applicants must have:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver’s license, reliable transportation, and a good driving record
- One year experience as a Mental Health Professional is preferred and experience working with children/adolescents is strongly preferred
This is a full-time position, primarily daytime hours of work Monday – Friday with two evenings/week and flexibility to accommodate unplanned client/family needs with little notice plus occasional on-call coverage after normal business hours. Competitive base pay, mileage reimbursement and great benefits are offered in this rewarding position. Pay range $24.60 – $25.83/hour (dependent upon experience).

~DIRECT CARE STAFF (DIRECT SUPPORT PROFESSIONAL)–Developmental/Intellectual Disability or Mental Health Services

As a Direct Support Professional at Transitions, you get to work with consumers who have intellectual or mental disabilities in group home settings, community settings, and/or in our day program. If you are patient and caring with a heart for helping others, this rewarding work might be for you! CNA experience is helpful; however, paid training is provided. Our evening/weekend positions feature competitive pay along with two bonuses paid during the first year! Pay range $18.00 – $23.00/hour, depending on shift and experience. Must be 18+ years of age and possess a High School Diploma/GED.

~TALENT ACQUISITION SPECIALIST
We’re looking for a Talent Acquisition Specialist to perform HR functions working with a staff of about 200 employees. Primary responsibilities are in the area of talent acquisition including candidate sourcing and advertising. applicant tracking, contact and engagement, scheduling and conducting interviews, checking references, conducting background checks, and leading orientation meetings. Will perform other duties within the scope of Human Resources or to support the agency’s employees and mission.
A Bachelor’s Degree in Human Resources, Business Management, Communications, or similar field of study is required; work experience in HR is preferred. Excellent communication skills, friendly personality, the ability to change gears/priorities quickly and to navigate multiple projects simultaneously are very important in this role. Full-time position, daytime hours of work, Monday through Friday. Competitive pay range of $23.58 – $25.94/hour (dependent upon education and experience) and excellent benefits await you in this position!

Our Youth CSWs make a real difference to children who have experienced a psychiatric or behavioral crisis by providing assessment and helpful supports, coordinating services with community providers, and teaching valuable skills.
