We believe, when possible, people should live in the least restrictive setting, close to family and friends. Transitions operates a number of group homes and apartments that provide safe and affordable housing to adults who have a serious mental illness or an intellectual/developmental disability. Our group homes provide 24-hour, supervised care to residents; those living in our apartments receive intermittent supportive services.
